Ariza Dawn Brooks, infant daughter of Jenifer Brooks of Fitchburg died Wednesday, May 13th in Heywood Hospital, Gardner.
Besides her mother, she leaves her maternal grandmother and step-grandfather, Tracy (Collette) and Craig Phelps of Ashburnham; one aunt, Veronica Phelps; three uncles, Craig Phelps, Jr., Philip Brooks II, and Matthew Brooks; and one cousin, Shane Francis Allerheiligen. She was predeceased by her maternal grandfather, Rodney Brooks of Templeton ,who died in 2004.
Funeral services are private and will be held at the convenience of the family.
Memorial contributions may be made to Heywood Hospital, Maternity Ward, 242 Green St., Gardner, MA 01440.
Boucher Funeral Home, Inc., 110 Nichols Street, Gardner, MA is handling the arrangements.
